Transitional Care Services Get You Back to Health and Home
When you or someone you love needs extra time and attention before heading home from the hospital, the transitional care team at Catholic Eldercare can provide the services you need for a speedy recovery.
Patients benefit from our intimate, welcoming setting with 24 private rooms directly adjacent to our rehabilitation department. Duchess Walker-Morgan, the nurse manager in our transitional care community, explains how it works. “With easy access to physical, occupational, and speech therapy right next door, evaluations happen quickly, and caregivers, family, and friends are close to support you in getting back home.”
Our experienced transitional care team works closely with you, your physician, and your hospital social worker to ensure you get the individualized care you need to recover quickly, safely, and effectively. “With state-of-the-art equipment, treatments, and therapies, we are fully equipped to meet the rehabilitative needs of patients with post-surgical, orthopedic, pulmonary, and neurological conditions and pain issues,” Duchess says. “We also provide wound care and dietary and medication management.”
In addition to therapies, rest is crucial for healing, so we want to make your stay with us as relaxing as possible. To that end, we offer a range of hotel-like amenities, including exceptional chef-prepared meals, daily housekeeping, and Wi-Fi and Cable TV in your room. Our transitional care guests can access our on-site bistro restaurant, beauty shop, and chapel.
